HIFU (high-intensity focused ultrasound) focuses ultrasonic waves at defined depths (1.5 / 3.0 / 4.5 mm) and is the only non-invasive energy technology capable of reaching SMAS depth. This enables superficial skin tightening together with deep-layer contraction. Improvement begins after a single session, but ideal collagen remodelling matures over 3-6 months. HIFU can deliver very satisfying results in middle-aged patients with moderate laxity and adequate bony support, but it does not replace surgical facelift in patients with advanced laxity, marked platysmal banding or significant skin excess. A common maintenance protocol is one session annually or every 18 months. Suitability is confirmed at consultation.

Morpheus8 is an RF microneedling device that delivers radiofrequency energy through needles into the deep dermis (up to 4 mm in head and neck areas). It addresses a broad set of indications: surface texture improvement (pores, acne scars), mid-layer contraction and sub-dermal fat reduction (for example, along the lower jawline). It can be used safely across Fitzpatrick skin types I-VI, an important advantage over many lasers. A typical protocol is three sessions 4-6 weeks apart, with results maturing over 3-6 months. Key indications include the neck, jawline, lower face and acne scars. The right needle depth, the right RF parameters and a suitable anaesthesia protocol are critical to outcomes.

Thermage FLX uses monopolar RF technology and aims at large-surface tightening in a single session; HIFU focuses ultrasonic energy at discrete points into deep tissue. Thermage produces broader, more superficial contraction; HIFU reaches deeper, down to SMAS. In practice the two are complementary rather than competitive: HIFU "pulls deep points upward," while Thermage "tightens broad surfaces evenly." For some patients a combination — for example HIFU followed by Thermage three months later — produces the most comprehensive result. Maintenance every 12-18 months is typical and is tailored to device and patient. The correct device is selected at consultation based on skin quality and target region.

Pain levels vary significantly by technology. Topical anaesthetic cream (5-10% lidocaine) is standard for RF microneedling, Morpheus8 and many fractional lasers, and after a 30-45 minute waiting time the treatment is well tolerated. With HIFU some zones (especially over bone) are more sensitive and oral analgesia or, rarely, local infiltration can be added if topical anaesthesia is insufficient. Modern Thermage generations include "comfort pulse" technology and can often be tolerated without topical anaesthesia. Cryolipolysis and EMS treatments do not require anaesthesia. Device treatment pricing depends on device amortisation cost, the number of single-use consumables (tips / cartridges) required, the size of the treated area, the number of sessions and clinic standards. HIFU transducer cartridges, for example, are patient-specific and have a limited number of shots; the size of the treated area therefore determines how many cartridges are needed and consequently the price. Morpheus8 / RF microneedling tips are also patient-specific. The price range is personalised to region and patient anatomy. When comparing "unlimited shot" or flat-rate offers, asking about the number of cartridges or tips used is the right question.
