"Doctor, I want to get a youth vaccine" — this sentence is perhaps the most common opening statement from my female patients aged 30-55 who come to my clinic. Then I ask in return: "Which brand? Profhilo, Skinbooster, NCTF, or Volite?" Most often, silence. This silence is the clearest indicator of how confused the "youth vaccine" umbrella term is in the Turkish aesthetic market. Because in reality, there isn't just one youth vaccine — there are at least six main brands and dozens of sub-formulations on the market, and each has different active ingredients, tissue levels, clinical indications, and expected results. In this guide, I will scientifically compare six frequently used brands in my clinic (Profhilo, Skinbooster/Restylane Vital, Juvederm Volite, NCTF 135HA, Cytocare, Sunekos); I will outline the criteria for choosing the right brand based on age, skin type, expectations, and budget. The term "youth vaccine" is a marketing-derived umbrella term; it has no exact equivalent in medical literature. In international dermatology publications, two main terms are used for this category: injectable skin boosters and bio-remodeling agents. These two concepts are not the same — and this difference is the first clinical distinction that determines which brand is suitable for which patient.

The primary goal of the skin booster category is to increase skin hydration, restore radiance, and improve the visual quality of the skin surface by placing stabilized hyaluronic acid (HA) in the upper-middle layers of the dermis. This category includes products such as Skinbooster (Restylane Vital and Vital Light), Juvederm Volite, and Belotero Revive. Their common feature: they contain cross-linked but low-viscosity HA.

The bio-remodeling category is different. In this group, pure, very high molecular weight (UHMW) hyaluronic acid is used; the aim is not just hydration, but to biologically restimulate the synthesis of type-I and type-III collagen and elastin in the dermis. Profhilo is the pioneer in this category, launched in 2015 with NAHYCO technology. Beleznay et al.'s 2017 study (J Drugs Dermatol) showed that Profhilo, unlike classic HA fillers, diffuses into the dermal matrix and directly increases fibroblast activity.

A third subcategory is polyrevitalizing / mesotherapy products: NCTF 135HA (Filorga), Cytocare, and Sunekos are included in this group. In these products, HA is not the sole active ingredient; vitamins, coenzymes, amino acids, minerals, or peptide chains are added. In the Turkish SERP, the search for "youth vaccine" most often triggers three misconceptions. These are:

• "Youth vaccine = filler" misconception. No. Classic HA fillers (Juvederm Voluma, Restylane Lyft) are placed at the subcutaneous or supraperiosteal level for volume restoration; the entire youth vaccine category works at the dermal level.
• "One session is enough" misconception. For all these brands, 2-3 sessions are required for the effect to settle; maintenance protocol is every 6-9 months.
• "They are all the same thing" misconception. The three categories above (skin booster, bio-remodeling, polyrevitalizing) are completely different in terms of clinical behavior, duration of effect, and patient selection.

Profhilo: Bio-Remodeling Pioneer

Profhilo (IBSA Farmaceutici, Italy) is a unique bio-remodeling agent with NAHYCO Hybrid Cooperative Complex technology, launched in 2015. Ideal Indication

In my clinical practice, Profhilo is the product I find most suitable for the following profile: patients aged 35-55, showing signs of dermal thinning, experiencing loss of skin quality and elasticity, but without significant volume loss yet. Profhilo's effect is particularly noticeable on the neck, décolletage, and back of the hands, in addition to the face — Profhilo stands out in these areas as there are few alternative bio-stimulants.

Standard protocol: 2 sessions, 30 days apart; first significant visual change in 4-6 weeks, peak effect in 60-90 days. Duration of effect is 6-9 months; a single session is applied every 6 months for maintenance. The side effect profile is low: the most common are papules (bead-like bumps) at the injection sites lasting 24-48 hours; these are characteristic of Profhilo and are part of the diffusion process. Ideal Indication

Restylane Vital is particularly suitable for the following profile: patients aged 30-45, with prominent skin dryness and dullness, fine lines, and not yet experiencing significant loss of elasticity. It is suitable for application on the face, especially the cheeks, periorbital area, and perioral area; on the body, the décolletage and back of the hands. Vital Light is preferred for sensitive areas (lower eyelid, front of the neck).

Protocol and Duration of Effect

Standard protocol: 3 sessions, 4 weeks apart; maintenance every 4-6 months. Duration of effect is 4-8 months (generally shorter than Profhilo). Side effects include edema, mild bruising, and small swellings at the injection sites lasting 24-48 hours. Juvederm Volite: 9 Months of Skin Quality with VYCROSS Technology

Juvederm Volite (Allergan/AbbVie, USA) is the product that brings VYCROSS HA technology to the skin booster category. VYCROSS is a system that cross-links high (90%) low molecular weight HA with low (10%) high molecular weight HA in a patented ratio; this allows the product to remain low viscosity but long-lasting.

Active Ingredient and Clinical Efficacy

Volite contains 12 mg/mL HA + 0.3% lidocaine. Its clinical advantage is that a single session can provide an effect lasting up to 9 months (multicenter trial, Niforos 2017, Dermatologic Surgery). This makes Volite stand out economically and in terms of patient compliance: compared to Restylane Vital, which requires 3 sessions, Volite can offer 6-9 months of skin quality improvement with a single session in most patients.

Ideal Indication

Volite is particularly suitable for the following profile: patients aged 35-50, with fine lines and loss of quality on the skin surface, who find frequent clinic visits impractical (busy professionals, frequent travelers). It is effective on the face, neck, and back of the hands. The micro-bolus technique of Volite produces good results on the cheeks and around the lips.

Protocol and Side Effect Profile

Typical protocol: 1 session + maintenance every 6-9 months. Side effects reported include post-injection redness, mild edema, and rarely late-onset papules (especially in thin skin). Late papules are a debated topic in the literature for VYCROSS products (Beleznay et al., 2017, J Drugs Dermatol). NCTF 135HA (Filorga): 55-Component Polyrevitalizing Agent

NCTF 135HA (New Cellular Treatment Factor, Laboratoires Filorga, France) is not a classic HA injectable; it is classified in the polyrevitalizing category. It contains unstabilized pure HA (5 mg/mL) along with 55 different components: 12 vitamins, 6 coenzymes, 23 amino acids, 6 minerals, 5 nucleotide bases, and 2 antioxidants. This multi-component formulation positions the product as the "gold standard of skin mesotherapy."

Mechanism of Action

NCTF is placed in the papillary dermis (1.0-1.5 mm) using mesotherapy technique (nano-needle, multi-needle adaptor, or individual micropapules). The goal is to stimulate fibroblasts with multi-factorial nutritional support. Lacarrubba et al.'s 2016 study (J Cosmet Dermatol) showed that NCTF 135HA demonstrated an objective increase in dermal echo-density after 4 sessions.

Ideal Indication

In my clinical practice, NCTF is my first choice especially for patients aged 30-45 with a "tired skin" appearance, experiencing skin dullness associated with stress and lack of sleep. It is also safer than Volite or Vital for treating fine lines in the under-eye area, perioral area, and décolletage, because the product does not contain stabilized HA and the risk of nodules is very low.

Side effects typically include small papules and mild redness at the injection sites lasting 24-72 hours. Late-onset side effects are rare. Cytocare (Revitacare): Age-Group Specific Matrix

Cytocare (Revitacare, France) is a polyrevitalizing series formulated specifically for different age groups: Cytocare 502 (young skin, 25-35 years), Cytocare 532 (middle age, 35-45 years), Cytocare 715 (advanced age, 45+). Clinical Positioning

The difference between Cytocare and NCTF is that Cytocare changes the HA density and revitalizing agent ratio according to the age group: 502 contains less HA and more hydration components; 715 contains higher HA and more intense revitalizing agents. In my clinical practice, Cytocare falls into the same indication pool as NCTF; the choice between them is often made based on patient skin analysis and sensitivity.

Sunekos: HA + Amino Acid Elastic Fiber Restoration

Sunekos (Professional Derma, Switzerland), launched in 2018, is a product claimed to have a different mechanism than other skin booster products. It contains low molecular weight HA (~200 kDa) with a patented complex of 6 amino acids (glycine, proline, lysine, alanine, valine, leucine). These amino acids are clinically the building blocks of elastic fiber synthesis.

Mechanism Claim

Sunekos's prominent clinical claim is that it directly stimulates dermal elastic fiber (elastin) synthesis, restoring skin elasticity through a mechanism separate from hydration. Cavallini et al.'s 2019 pilot study (Aesthetic Surgery Journal) presented findings supporting this claim with elastometer measurements over a 12-week follow-up; however, the literature is not yet as established as for NCTF or Profhilo.

Ideal Indication

In my clinical practice, Sunekos is considered especially for patients aged 38-55 with prominent loss of elasticity in the lower eyelid, perioral area, and décolletage. It has an overlapping indication pool with Profhilo; the choice is often made based on patient skin structure, budget, and previous treatment history.

Protocol and Duration of Effect

Standard protocol: 3-4 sessions, 10-14 days apart; maintenance every 4-6 months. Duration of effect is 5-7 months.

1. Age Group

25-29 years: Generally, a youth vaccine is not necessary; skin routine (SPF + retinol) and hydration are sufficient. Primary goal hydration → Restylane Vital, Volite. Primary goal elasticity and texture → Profhilo, Sunekos. Primary goal multi-nourishment and "tired skin" → NCTF, Cytocare. These three goals are often mixed; which one is dominant is clarified during consultation.

4. Area

Face: All brands can be applied. Neck and décolletage: Profhilo is the first choice. Lower eyelid: NCTF or Sunekos (low nodule risk). Back of the hand: Profhilo or Volite. Scalp (in combined alopecia treatment): NCTF Hair or hair-specific mesotherapy formulations.

5. Side Effect Tolerance and Recovery Time

For patients who travel frequently / have planned social events like weddings: Volite is more practical with a single session. For patients who cannot tolerate the risk of late papules, Profhilo (BAP papules disappear in 24-48 hours) or NCTF is preferred. Cannula technique is used to reduce the risk of bruising in patients with an active social life.

6. Clinical Contraindication Profile

Common contraindications for all these brands: pregnancy, breastfeeding, active skin infection (especially herpes), history of angioedema, active autoimmune disease, isotretinoin use in the last 6 months, anticoagulant therapy (relative), keloid tendency. Lidocaine allergy is an absolute contraindication for lidocaine-containing products (Volite).

7. Budget and Maintenance Sustainability

A youth vaccine is not a one-time investment; it is a continuous protocol requiring periodic maintenance sessions every 6-9 months. During consultation, the patient's budget framework that can be sustained within a 12-24 month plan is discussed; a sustainable "mid-segment" plan that can be continuously applied produces clinically more valuable results than an unsustainable "upper segment" plan. Hyaluronic Acid Molecular Behavior: Key to Understanding Brands

Above, we examined six brands individually; however, to understand these brands in depth, it is also necessary to know the molecular behavior of hyaluronic acid. Hyaluronic acid (HA) is a polysaccharide structure composed of N-acetyl glucosamine and glucuronic acid sequences, found in the natural dermal matrix. Approximately 5 grams of HA are synthesized and the same amount is broken down in our body daily; this balance is disrupted with age. Three basic parameters determine the clinical behavior of an HA injectable: (1) molecular weight (kDa), (2) cross-link ratio, and (3) HA concentration (mg/mL). Profhilo's hybrid structure of H-HA (1,100-1,400 kDa) and L-HA (80-100 kDa) is non-cross-linked; therefore, it diffuses rapidly into the dermis. Restylane Vital, on the other hand, is stabilized with a low degree of cross-linking; HA remains in the tissue longer but diffuses less. VYCROSS technology (Volite) combines two molecular weights in different ratios with a patented cross-link profile to provide long-lasting and low-viscosity results. These three different approaches explain why clinical results are so different.

Polyrevitalizing products like NCTF and Cytocare contain unstabilized HA (generally < 8 mg/mL). This allows non-cross-linked HA to interact more quickly with the dermis and for added nutrient components (vitamins, amino acids, minerals) to rapidly reach fibroblasts. Sunekos, on the other hand, takes a different approach, positioning HA as a "carrier" for its patented amino acid complex. These molecular differences show that the question "which brand is stronger" is incorrectly framed; the correct question is "which brand is optimized for which target in which layer." The most misunderstood aspect of the youth vaccine category is its marketing as a "one-session solution." In reality, all brands require periodic maintenance sessions to sustain the effect. An annual plan approach optimizes both clinical results and budget.

Season-Based Strategic Approach

In my clinical practice, I plan youth vaccine programs in alignment with the seasons — this is important for both skin physiology and patient lifestyle. Spring (March-May): the period when the skin is most open to renewal after winter; ideal for initiating main sessions. Summer (June-August): due to high UV intensity, maintaining the existing effect, strict SPF protocol, and maintenance sessions are preferred instead of starting new series. Unfortunately, with the "youth vaccine" trend in the Turkish market, some clinical application errors have also become widespread. As Dr. Hamza Gemici, I consider it my responsibility to share these errors for patient safety:

• Not asking for the product's lot number and origin. There is a risk of parallel import and counterfeit products in Turkey; patients should demand to see the original box, lot number, and certificate of origin.
• Too rapid session intervals. Performing Profhilo sessions 2 weeks apart is off-protocol; 4 weeks is recommended in the literature. Rapid sessions do not increase efficacy but rather increase the risk of side effects.
• Incorrect layer application. Injecting NCTF subcutaneously or into the deep dermis; leaving Profhilo as a superficial papule — both reduce efficacy and increase the risk of nodules.
• Mixing multiple brands in the same area in the same session. As stated in Dr. Gemici's note above.
• Misleading the patient with the misconception that "one session is enough." No brand in this category provides maximum results with a single session; a maintenance plan is critical.
• Not performing contraindication screening. Injecting without screening for pregnancy, autoimmune disease, or isotretinoin use is a serious ethical violation and clinical risk.
