Botox and filler are often mentioned together, but they do different jobs. Botox reduces unwanted muscle movement, while filler restores structure and volume.

Choosing correctly depends less on trends and more on anatomy. I decide based on whether the main issue is dynamic lines, volume loss, or a combination of both.

1. What Botox Actually Treats

Botox relaxes targeted facial muscles. It is best for expression-related lines such as the forehead, glabella, crow’s feet, and selected lower-face or neck indications when used carefully.

It does not fill hollow areas or replace lost support. If the wrinkle remains visible even when the face is at rest, Botox alone may not be enough.

2. What Filler Actually Treats

3. When One Treatment Is Enough and When Combination Makes Sense

A younger patient with early forehead or frown lines may only need Botox. A patient with flatter cheeks, lip deflation, or a weak chin may need filler instead. Many patients, however, have both movement-related lines and structural volume loss.

In those mixed cases, combining treatments often creates a more balanced result. Botox reduces the folding force, and filler restores support where the face has lost shape.
