Superficial compartments include nasolabial fat, jowl fat and lateral malar fat; deep compartments include deep medial cheek fat, suborbicularis oculi fat (SOOF), retro-orbicularis oculi fat (ROOF) and buccal fat. With ageing these compartments shrink, descend and shift in an uneven pattern, producing the "sliding downwards" appearance that is in fact a combination of volume loss and migration. Modern mbushes strategy uses a supraperiosteal (on-bone) plane with firm, high-G' HA or biostimulator mbushes such as CaHA to mimic this lost bony support. This approach provides structural stability and a foundation for redistributing the more superficial soft-tissue compartments. Superficial mbushes alone, in the absence of underlying support, can produce a "puffy but unstructured" result — which is why an anatomical approach matters.

At Dr. Hamza Gemici’s clinic every injectable patient undergoes a standardised pre-trajtim routine: standardised lighting photography (frontal, three-quarter, profile, chin-up/chin-down), animated mimetic photographs, anatomical landmark marking, vascular safe-corridor planning and ultrasound guidance where required. This mapping increases safety and allows the trajtim plan to be reviewed jointly with the patient. At follow-up the same positions are repeated for objective krahasim. Rapid "straight to injection" consultations are not the recommended standard.
