An artificial appearance is rarely caused by the product itself; it is mostly a planning problem. Overloading a single region (typically lips or cheeks), mismatching product and tissue plane, or superficial placement without addressing underlying bone support tend to create unnatural results. Conservative dosing spread across several sessions produces both more natural and safer outcomes. Avoiding "Russian Lips" or visibly overfilled looks, and matching product to anatomy, are the main safeguards against an artificial appearance.

The longevity of hyaluronic acid fileri depends on cross-linking density, gel firmness (G'), the region treated and patient metabolism — typically 6-24 months. Mobile zones (lips, perioral area) average 6-12 months; deeper structural areas such as the cheekbones, chin and jawline can hold 12-18 months; very stable supraperiosteal placements may last up to 24 months. Biostimulator fileri (CaHA, PLLA, PCL) provide direct volume plus collagen induction, with an indirect durability of around 18-24 months. The exact personal timeline depends on product choice and treated region; an annual review with optional touch-up is a common maintenance plan.

Post-filer oedema (swelling) and ecchymosis (bruising) are expected, transient reactions. In mucosa-adjacent regions such as lips, swelling peaks within 24-72 hours and substantially settles within 5-7 days; the final result is evaluated at day 14. Bruising can be minimised with cannula technique and careful injection but cannot be eliminated entirely; it typically fades within 5-10 days. To reduce swelling we recommend cold compresses in the first 48 hours, sleeping with the head elevated, avoiding salty / alcoholic / spicy foods and avoiding intense exercise. For important social events, scheduling tretman at least 14 days in advance gives time for swelling to resolve and the result to settle.

There are clear contraindications to filer tretman. Active skin infection at the injection site (including herpes simplex), pregnancy and breastfeeding, uncontrolled autoimmune disease, known allergy to product components, coagulopathy or untreated bleeding disorders and certain immunosuppressive regimens are absolute or relative contraindications. Tretman is also deferred for patients with recent dermal laser or peel procedures, planned dental surgery or implants, or unrealistic expectations and suspected body dysmorphic disorder. Adding HA filer to areas previously treated with permanent (non-resorbable) fileri requires especially careful planning. Determining candidacy remotely without a detailed history and examination is not safe practice.

Filer pricing depends on the type of product (HA / CaHA / PLLA / PCL), the brand, the number of millilitres used, the number of treated regions, physician experience, clinic standards and ancillary materials (cannulas, topical anaesthesia, cooling). Two patients treated in the same region but needing 1 ml vs 3 ml will naturally have different costs. Therefore the price range is personalised to region and anatomy. At our clinic the price is shared in writing alongside the tretman plan after consultation, once product type and quantity have been decided. "Flat-rate" filer campaigns should be approached with caution because correct tretman is product-and-dose specific, not campaign-driven.
