Side Effects and Management: What and How to Observe?

Botox is a safe procedure; however, no medical intervention is completely without side effects. This section describes possible side effects, their frequency, and when to call a doctor with a clear guide.

  • The most common side effect is temporary bruising/tenderness at the injection site; it resolves in 24-48 hours.
  • Eyebrow or eyelid drooping occurs in 1-3% of cases; it recovers in 4-6 weeks.

Common and mild side effects

The most common side effects are: mild bruising (ecchymosis) at the injection site, temporary redness, tenderness, small pinpoint bleeding. Almost all of these effects disappear within 24-72 hours and do not require treatment. Applying ice and skin protective creams provides relief. If bruising occurs, arnica or heparin-containing gel may help after 48 hours.

Mild headache (especially after glabella) is seen in 10-20% of patients in the first 24 hours and improves with paracetamol. Small changes in bowel movement or sleep quality may be reported on the first day after injection; these are usually due to placebo or attention to treatment, and do not constitute a clinical finding.

Moderate side effects

Moderate side effects: brow ptosis, eyelid ptosis, asymmetry, crooked smile. These occur if the injection site is chosen incorrectly or the dose is too high. They are seen in the range of 1-5%. In eyelid ptosis, apraclonidine 0.5% (ionopin) eye drops partially help; these drops stimulate the eyelid elevator and lift the lid by 1-2 mm.

The "treatment" for these side effects is mainly time. As muscle activity returns, the problem subsides and resolves completely within 4-8 weeks. The patient should not panic, should inform their clinic and make a follow-up appointment. Additional needle recommendations during this process are usually wrong — when "let's fix it with a needle" is said, it is observed that a second mistake does not correct the first.

Attention:For the first 4 hours after injection, do not massage, take a hot shower, or lie face down. These rules reduce the risk of the toxin spreading to unwanted areas. Always get written instructions from your clinic and follow them.

Rare and serious side effects

Rare but serious side effects: generalized muscle weakness, difficulty swallowing (dysphagia), voice change (dysphonia), difficulty breathing, allergic reaction (rare). These symptoms may occur if botox spreads and affects distant muscles. It is usually associated with high doses and is extremely rare at aesthetic doses.

If new onset difficulty swallowing, voice loss, or arm/leg weakness occurs 1-10 days after injection, an emergency room should be visited. In this case, a therapeutic antibody (antitoxin) can be vital. All of these conditions occur at a rate of 1-2 per ten thousand per lifetime application; however, they should be known.

When should you call the clinic?

  • If your eyelid has significantly dropped and is interfering with your daily activities
  • If your smile has become asymmetrical
  • If you start experiencing difficulty swallowing, hoarseness, or difficulty breathing (emergency)
  • If bruising still does not decrease and spreads after 72 hours
  • If there is fever, swelling, or discharge at the injection site (suspicion of infection)
  • If you experience generalized muscle weakness or double vision (emergency)

Is there a risk of long-term side effects?

The long-term safety of regular botox application is supported by 30+ years of data. Permanent muscle damage, systemic toxicity, or cumulative harm have not been documented. However, in very long-term (20+ years) and high-dose applications, muscle atrophy (permanent thinning) may occur; this is already a desired result in aesthetic masseter botox.

Antibody development (resistance) rarely occurs and is usually due to a combination of high dose + short interval (less than 3 months). If the effect starts to decrease in a patient, product change (Botox → Xeomin, preservative-free formula) and lengthening the intervals may help.
