Understanding Dermal Fillers Correctly

The balance of volume, proportion, safety, and naturalness in filler applications.

Main ideas in this chapter

  • Fillers are used to regulate proportion and support as much as to add volume.
  • Good filler works invisibly; it improves the balance of the face.
  • The biggest mistake is trying to directly fill every hollow.

What is filler?

Dermal filler is a medical application mostly used with hyaluronic acid-based products to provide volume support, regulate certain contours, and establish structural balance in some facial areas.

However, filler is not only done to inflate a specific area. The best filler creates a more balanced, softer, and more natural look on the entire face.

Where is it most often misunderstood?

The patient sometimes directly targets the line; however, the source of the line may be elsewhere. The source of the under-eye shadow may be mid-face support, and the reason for the stiffness around the mouth may be the lower face ratio.

Therefore, a good physician considers the filler not only for the area that appears deficient but also according to the structural plan.

  • Volume support
  • Contour softening
  • Balancing facial proportions
  • Creating natural transitions

Rule of measure:Quality does not increase with the amount of filler. Most often, the best result is achieved with the most invisible but most accurate amount.

What should the patient know?

Filler should not produce a permanent change in identity. The goal is not to transform a person's face into another form but to refine tired or unsupported areas.

Correct candidate selection, product selection, anatomical knowledge, and the decision to forgo the procedure if necessary are the foundations of filler safety.
