Skin longevity is the science of managing not "how many years the skin has lived," but "how well it has lived." This section explains the difference between chronological age and biological age, epigenetic clocks, and why a 35-year-old's skin can look like a 25-year-old's.

Where does the term "skin longevity" come from?

The word "longevity" means "long life"; in medicine, it refers not just to living long, but to living well. A person reaching 90 years old sick and dependent is not the same as reaching the same age with energy, clear thought, and full function. The situation is exactly the same for the skin. "Skin longevity" does not mean the skin wrinkles more slowly — it means it ages healthier, gets sick less often, and repairs itself better.

As Dr. Gemici, I differentiate the skin longevity approach from other aesthetic approaches in my clinic. "Anti-aging" is a declaration of war — against time. Skin longevity, on the other hand, is a proposal for collaboration: moving forward in harmony with time instead of trying to stop it.

Calendar age vs. biological age

The age written on your ID card is your chronological age and it does not change. But your cells, tissues, and organs can be at a different "biological age" than this chronological age. Of two siblings who are both 45, one might have aged like a 35-year-old, while the other might have aged like a 55-year-old. The factor that creates this difference is the epigenetic program of the cells and the balance of damage-repair.

Developed by Dr. Steve Horvath in 2013, the "epigenetic clock" (Horvath Clock) can calculate the biological age of cells by reading methylation patterns on DNA. Studies conducted after 2019 have shown that this epigenetic age can be reversed with lifestyle and medical interventions.

Clinical note:Today, there is no practical way to measure the biological age of the skin; however, we can assess the functional age of the skin visually and with measurements: water retention capacity, elasticity, pigmentation homogeneity, pore appearance, and micro-vascular health together form a "skin age" score.

What accelerates aging? (Hallmarks of Aging)

In 2013, the scientific community identified 9, and in subsequent years, 12 fundamental mechanisms of aging. The most critical ones for the skin are: (1) genomic instability — UV and free radical-induced DNA damage; (2) telomere attrition — shortening of chromosome ends with cell division; (3) epigenetic alterations — shift in DNA methylation; (4) loss of proteostasis — deterioration of collagen-elastin protein quality; (5) mitochondrial dysfunction — decrease in cellular energy; (6) cellular senescence — accumulation of "zombie cells"; (7) stem cell exhaustion — decrease in regenerative capacity.

Each of these mechanisms triggers the others. When one point breaks down, the others also get involved in the process. A skin longevity strategy requires a multi-agent approach that targets these mechanisms individually — a single cream or procedure is not enough.

Why is it important to "start in your 20s"?

Many patients start thinking about skincare when the first noticeable wrinkle or spot appears — this is usually between 35-40 years old. However, the structural elements of the skin (collagen Type I, Type III, elastin, hyaluronic acid) begin to decrease by approximately 1% per year after the age of 25. This loss continues for 10-15 years before becoming visible. At 35, the first wrinkle is not a "beginning," but the visible face of 10 years of silent accumulation.

In Dr. Gemici's Protocol, I recommend a minimal skincare routine starting from age 25, and a structured one from age 30. This early intervention can reduce the need for laser or thread lifts at age 45 by 40-60%. But let's say you just turned 50 and haven't started at all — a significant improvement of 5-10 years is still possible; I observe this every day in my clinic.
