PRP and Exosome: The Science of Regenerative Therapies

PRP is derived from the patient's own blood; exosomes are the signaling balloons of cells. This section evaluates the entry of regenerative medicine into the aesthetic field, the difference between the two approaches, and the level of evidence.

The biology of PRP

PRP is separated from approximately 20 cc of the patient's blood via centrifugation. Platelets are separated, and a plasma concentrated approximately 3-5 times is designed. Platelets carry growth factors in their granules: PDGF (platelet-derived growth factor), TGF-β, VEGF, EGF, IGF. When injected into the skin, these factors activate fibroblasts, keratinocytes, and microvessels; they increase collagen synthesis and improve microvascularization.

In my clinic, I find PRP particularly effective in combination with microneedling. Microneedling opens channels, PRP is absorbed into these channels, and cells receive direct signals. It is applied for indications such as wrinkles, fine lines, skin texture, scars, and alopecia (hair loss).

Exosome: next-generation regeneration

Exosomes are small vesicles, 40-150 nm in diameter, secreted by cells. They carry mRNA, miRNA, proteins, and lipids inside them. They are couriers that carry "packaged messages" from cell to cell. Exosomes derived from stem cells have the potential to transmit regenerative signals without sending the stem cell itself (without creating a cancer risk).

Exosome applications, which have become widespread in aesthetics after 2020, are generally produced in laboratories from umbilical cord mesenchymal stem cells. They are used for indications such as skin radiance, inflammatory skin diseases (rosacea, dermatitis), hair regrowth, and accelerating post-procedure healing.

Attention:The standardization of exosome products is still controversial. Each manufacturer uses different sources and different isolation methods. The FDA (USA) currently classifies exosome products as "unlicensed biological products" and applies some advertising restrictions. In Turkey, the licensing process continues to mature. Reliable clinics use ISO-certified production facilities and products with traceable lot numbers.
